SWCA-AZ sees patients by referrals only. If you are a patient who would like to be seen at SWCA-AZ, please ask a member of your care team to fill out a referral form and fax to the number listed on the form. Referral sources can any member of your care team such as home health, primary care, podiatry, vascular surgery etc.
